Certificate of Incorporation

Upload a clear copy of the CAC incorporation certificate.

Application Letter with Passports

Include the application letter and passport photographs in one document.

Valid ID

Driver’s licence, voter’s card, or other valid identification.

Form C30

Upload a clear copy of Form C30.
